FM-30 Forms
Overview
A FM-30 form is required for the following:
- New vehicle request
- Changes in assigned driver or custodian
- Request to change type of vehicle
The FM-30 requires three signatures: the driver, department head, and agency fleet coordinator. To prevent any delays in processing, all signatures must be obtained prior to submitting your request.
Assignment Types
Individual Assignment
An individual assignment is designated when only one person will be driving the vehicle. The vehicle is not for shared use. An individual may only have one designated individual assignment.
Agency Assignment
An Agency assignment is designated when more than one individual will be driving the vehicle. The individual listed on the FM-30 is considered the vehicle custodian and is responsible for the vehicle(s) assigned. An individual may have multiple designated agency assignments.
Vehicle Changes
When requesting to change your assigned vehicle, please include the current vehicle number in the justification/notes section and the type of vehicle you are requesting. Please note there will be an increase in rates for any vehicle upgrades.
